Health Teams
At CVFP, patients have access to integrated and comprehensive care delivered by 5 health teams - Gold, Silver, Green, Yellow and Blue. These teams include Family Doctors, specialty health providers like Pharmacists, Registered Nurses and Medical Office Assistants.
Communication is critical so our Health Teams “huddle” every morning before patients arrive to look at and discuss the day ahead. The huddle gives teams the opportunity to plan daily tasks and roles, and optimize well-coordinated patient care.
Rainbow Team
In 2024, CVFP launched its Rainbow Team – a model that embodies team-based care innovation. This new care model positions CVFP to navigate the family doctor shortage by integrating health providers including registered nurses and physician assistants working to their full scope. By empowering these health providers as key contributors in team-based care, CVFP family doctors are able to focus their time as mentors, consultants and guides for complex cases.

Physician Assistants
A physician assistant (PA) is a healthcare professional who works in collaboration with a physician to deliver high-quality patient care. In CVFP PAs work in the clinic setting alongside family physicians. These health providers function very much like a family doctor would: they assess patients, order and interpret labs and imaging, diagnose and create treatment plans, provide patient education, prescribe medication, and perform in-office procedures. PAs work on behalf of their supervising health team physicians and are often thought of as extensions of the physicians. By integrating PAs into primary care, clinics can improve wait times, clinic efficiency and workflow, as well as increase patient satisfaction and reduce overall costs.

Registered Nurses
CVFP Registered Nurses work with your doctor as part of your integrated Health Team. They may be part of your CVFP in-person appointment, and they may also call you to review results, complete health teaching or talk to you about symptoms you have.
Our Triage Nurse team sees patients in clinic for various procedures and injections, as well as calls patients who are experiencing certain symptoms.

Preventative Care
“Seeing your CVFP doctor for targeted preventive screening is important. This does not necessarily mean doing annual physicals with tests. We believe that preventive care to improve overall health should be personalized and involve shared decision-making between you and your CVFP health team.”

Uninsured Services
CVFP is publicly funded by Alberta Health and patients do not pay for the cost of routine medical care. However, CVFP does charge for services which are not medically required and are not covered by provincial health insurance. Fees for these uninsured services follow guidelines from the Alberta Medical Association and include sick notes and insurance requests.
CVFP offers multiservice subscription options for uninsured services as follows:
Family or Single
1-Year Subscription: Family ($125.00) OR Single ($65.00)
2-Year Subscription: Family ($230.00) OR Single ($115.00)
✓ Notes (sickness/absence, physio, massage, chiropractic, orthotics, compression stockings) *without the service the cost is $30 per note
✓ Driver’s forms for medical reasons (does not include other forms) *without the service the cost is $100 per form
✓ Photocopying of medical results *without the service the cost is $2 per page
Seniors: age 65 and over
1-Year Subscription: Per person ($50.00) OR Couple ($85.00)
2-Year Subscription: Per Person ($90.00) OR Couple ($165.00)
